UPI works beautifully until it does not, and then it fails in a handful of predictable ways. The good news is that almost every UPI problem has a known cause and a two-minute fix — the trouble is that the error messages rarely tell you which one you are looking at.
This is a diagnostic guide. Find your symptom, get the cause, apply the fix.
1. “Transaction Failed” With No Money Debited
Cause: a timeout somewhere between your app, your bank and NPCI. Most common at month-end and around midnight when banks run settlement jobs.

2. Money Debited But Not Received
Cause: the transaction is stuck awaiting settlement.

3. “Incorrect UPI PIN” When You Know It Is Right
Cause: you are paying from a different bank account than the one that PIN belongs to. The PIN lives with the account, not the app.

4. “Limit Exceeded”
Cause: you have hit either the daily value cap or the roughly 20-transactions-per-day count limit that most banks impose.

5. Cannot Send More Than ₹5,000 to a New Contact
Cause: the new-payee cooling period, a standard anti-fraud control.
Fix: none available — it lifts after 24 hours. Plan ahead by adding the payee and sending ₹1 a day early.
6. “Unable to Verify SIM” During Setup
Cause: the app cannot send its verification SMS from your registered number.
Fix: put the right SIM in slot 1 on a dual-SIM phone, grant SMS permissions, turn mobile data on rather than relying on Wi-Fi, and make sure a prepaid SIM has enough balance to send an SMS.
7. Bank Account Not Showing Up
Cause: the mobile number in your phone is not the one registered with that bank.
Fix: update your mobile number at the bank branch or through net banking. There is no way around this — it is the foundation of UPI’s identity model.
8. Payments Fail Only With One Merchant
Cause: the receiving side — an inactive UPI ID, a frozen merchant account, or the receiving bank being down.

9. UPI Works on One App But Not Another
Cause: an app-level cap, a stale app session, or a caching issue.
Fix: update the app, clear its cache, or re-link the account. If it persists, use the app that works — the differences between UPI apps are smaller than people assume, and keeping two installed is the real solution.
10. AutoPay Mandate Failed
Cause: insufficient balance at the moment of debit, or an expired mandate.

11. Unexpected Deduction After a UPI Payment
Cause: almost always a bank charge rather than a UPI charge — a minimum balance penalty or a quarterly SMS alert fee.

12. Everything Fails, Repeatedly, All Day
Cause: your bank is having an outage. This is more common than banks like to admit, particularly on the 1st and the last day of the month.
Fix: use a second bank account. If you only have one, UPI Lite keeps working during bank outages because it pays from an on-device balance. This is the single strongest argument for setting it up before you need it.
The Five-Minute Setup That Prevents Most of This
- Link two bank accounts at two different banks.
- Install two UPI apps, one on each account.
- Enable UPI Lite with a small balance for outage days and small payments.
- Know your daily limit before the day you need it.
- Turn on app lock and keep your default account’s balance modest.
That combination removes the great majority of UPI failures people experience, and it takes about five minutes once.
When It Is Not a Technical Problem
